The order form, explained

The Order Form is the page you sign. One Adobe Acrobat Sign signature accepts it, the Standard Terms and the Data Processing Addendum, plus a Business Associate Agreement for healthcare accounts.

1. Order details. Your campaigns and collectors, and this scope: "Every campaign and agent listed above, up to 500 agents, assessed together in one Floor Assessment. A larger floor is quoted before signature. The fee is the same however many campaigns are included, and anything added later needs its own Order Form."
2. Stage 1. Your report, and one assessment: "The fee covers one Floor Assessment: one data delivery under section 3, one report and one readout. A further or refreshed assessment, or any campaign, team, site or business unit not listed in section 1, needs a new Order Form before it is assessed or moves to Stage 2, because its models are built from its own data."
3. What you provide. Four data files and a data contact.
4. Fees and payment. $15,000, due within 7 days, before any data moves.
